The current implementation of fused attention concatenates the g_idx of QKV, which doesn't make sense because QKV are concatenated in the output dimension while g_idx is only relevant to the input dimension. Although this implementation is functional, it is not logically correct.
Also fix a typo in setting max_input_len.
